Ban Request Form

I think its time to update the Ban Request Form and procedures. From my experience anyways the Ban Request Form itself is full of fail and needs to be either updated or removed entirely. Most people simple post the players name/steam id/reasons in their post for a ban anyways.
Thoughts?
